TITLE: EHS HOME BASED EDUCATOR

BASIC FUNCTIONS: The Home Based Educator shall serve as the pregnant woman’s or family’s primary contact with the La de Esperanza Early Head Start Program.  The Home Based Educator is responsible for developing a relationship with family in the home and community to build upon strengths. Services should include screening and assessment, medical, dental, and mental health services, child development and education and family partnership planning.

FLSA: Hourly – Non-exempt

POSITION RESPONSIBILITIES:

  1. Manage a caseload of 10-12
  2. Participate in intake, assisting families to complete all necessary documentation.
  3. Collaborate with families to develop Family Partnership Agreements, identifying family strengths, involving parents in recognizing their abilities, and helping parents develop their own strategies for meeting goals and caring for their children.
  4. Visit families in the home one time each week for 1.5 hours. Implement the Parents as Teachers curriculum.
  5. Provide documentation of home visits.
  6. Plan and facilitate two socialization sessions each month in cooperation with other home based educators.
  7. Submit socialization reports and planning form, including menus.
  8. Assist families to secure prenatal and/or postnatal services, referrals and appointments.
  9. Assist first-time parents
  10. Assist parents to access services and coordinate services to meet the family’s needs.
  11. Advocate for families.
  12. Maintain records of FPAs, lesson planning, child assessment, socializations, and all follow-up.
  13. Ensure implementation of services outlined on IFSPs.
  14. Assess family needs. Develop strategies to address identified needs through programs, services and other resources at La Casa and in the community.
  15. Assist in the transition of three-year-olds to Head Start or other child development programs, as determined by family choices and needs.
  16. Coordinate with Health Services staff to ensure that post-partum visits are conducted.
  17. Assist all families with disabilities/mental health concerns in receiving all available services.
  18. Encourage family participation and input into all program activities.
  19. Participate in reflective supervision sessions.
  20. Maintain confidentiality at all times.
  21. Obtain a minimum of 15 hours of professional development each year.
  22. Submit reports as requested.
  23. Perform other duties as assigned.

SKILLS AND QUALIFICATIONS:

  1. 1-3 years of professional experience in service coordination, counseling, early childhood education or childhood development.
  2. Bilingual (English/Spanish) preferred.
  3. Shall be at least 18 years of age.
  4. Diplomacy and human relations skills.
  5. Knowledge of, or ability to quickly learn about services for pregnant women, infants, toddlers and their families.
  6. Must be reliable and flexible to meet the needs of the program.
  7. Ability to work as a team member.
  8. Ability to work effectively with diverse families.
  9. Ability to work in a fast-paced environment.
  10. Excellent written and oral communications skills.
